Working with a large team of Care Navigators across three surgeries you will be the first point of contact for our patients. You will be working alongside our clinical and administration staff ensuring that we offer an excellent level of patient service. You will be good listener, be caring and empathic, and have clear communication skills both face to face and using the telephone. You will be guided by the site Supervisors and Team Leaders; you will work in accordance with the practice guidelines and protocols. The post-holder will be responsible for navigating our patients to the correct type of appointments and services the surgery has to offer.

Duties and responsibilities:

  • Processing personal information and arranging requests for appointments, visits and telephone consultations.
  • Take in documents from patients and distribute to the appropriate teams
  • Deal with incoming telephone calls, act accordingly, or take accurate messages and pass on to the relevant personnel
  • Enter data into patient’s electronic NHS Care Record
  • Action daily Tasks posted on the practice clinical system
  • Register new patients on the practice clinical system
  • To generate EMIS patient online access
  • To book voluntary transport or hospital transport
  • To book emergency ambulance when asked to do so by a clinician

Person Specification:

The position holder should have the following attributes:

  • Be able to empathise and be understanding.
  • To have basic computer skills
  • To have a good telephone manner
  • To be able to work independently when needed.
  • To remain clam in a busy environment
  • To be a great communicator and have good listening skills
  • To have had experience of working with the general public
  • Experience of working in a GP’s surgery is desirable not necessary, as full training will be given to the right candidate.

The Swan Practice is looking to recruit an additional nurse to their amazing team of healthcare professionals.

We are looking for a  Long Term Conditions Nurse who wishes to develop their clinical expertise as an autonomous practitioner in an exciting and challenging Long Term Conditions Practice Nurse role.

 

Using your extensive relevant qualifications, skills and experience you will hold responsibility for working with patients with multiple long term conditions requiring intensive support and management.

This will be carried out with close supervision, mentorship and support by our Nurse Team Manager and very supportive and dynamic group of GP’s.

You will develop a proven ability to make high level clinical decisions that will aim to prevent avoidable hospital admission for this group of patients.

You will also develop exceptional management and communication skills which will enable you to work closely with GPs, community nursing teams and other care professionals to establish and maintain a whole systems approach for these patients.
