There are several ways you can contact us to discuss your medical concerns.
Contact us online via our website. We provide an online consultation service called Klinik whereby you complete a short form detailing your medical concerns, your request will be triaged, and you will be contacted by the most appropriate clinician.
You can call the surgery between 08:00 – 18:00. Please note that call times may be longer during our busier times.
As a parent/guardian, you may wish to have Proxy Access via your online service provider i.e NHS App, which will allow you to book an appointment or order their repeat medications. Full details including the online application forms can be found on our website.
Proxy Access – The Swan Practice
All our patients now have a named accountable GP, who is responsible for patients’ overall care at the Practice. Where a patient expresses a preference as to which GP they have been allocated, the practice will make reasonable efforts to accommodate this request. Please be aware that this does not mean you can only see your named GP. Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not always be available and if your needs are urgent, you may need to discuss them with an alternative doctor.
