How to Register
You will need to complete a registration form including health details which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Please complete the registration online at https://gp-registration.nhs.uk/K82007 or through the NHS App.
If you do not wish to, or are unable to register online, please come to North End, Verney Close or Steeple Claydon surgery after 11.30am to register.
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
Named Accountable GP
You will be assigned to a doctor who will be your “Named Accountable GP” as required by the NHS, however, you can choose to be seen by any clinician as long as they are available. You can ask to be seen by any of the doctors in the practice although it will not always be possible to see the doctor of your choice on a particular day and time.
Practice Catchment Area
If you live within our Practice Area you are welcome to register with us.
Please enter your postcode to check that you live within our catchment area, before you register. Please also let us know as soon as possible if your address or home or mobile telephone number changes.
It is important that if you have changed address, you first check our practice catchment to see whether you are still living within the boundary. If you are now outside of our catchment area, please register with a more local practice. This is to protect your own healthcare needs in the event that you need services such as home visits etc.
Check your Postcode
Please note that every care has been taken in the preparation of information on this Postcode Mapping Tool, however there may be instances where the Postcode Mapping Tool does not include your location even though it is within our Practice Boundary. An example of this may be new housing developments where the postcodes are not yet included on Google. If you have any questions please contact reception.

People of No Fixed Abode
If you are homeless or of no fixed abode but reside within the practice boundary, then you can still register with us.
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
