 Practice Nurse

Location At: Any of our three sites in Buckingham or Steeple Claydon

Contract type: Full time Permanent

Hours per week: 37.5 hrs per week

Pattern: To be worked over a 5 day week ( negotiable)

Requirements: Own car

Payscale: Grade 5

The Swan Practice are looking to recruit an enthusiastic and experienced Practice Nurse at their busy NHS GP practice.

The successful applicant must have a good range of skills and be able to work on their own initiative. Good communication skills are essential .

The Swan Practice use the EMIS web computer system and provide high quality care for their patients.

Currently the practice offers the following service but training can be given for the right candidate

Please see below some of the clinical requirements:

· Smear taking

· Wound care

· ECG’s

· Asthma Clinic

· COPD Clinic and spirometry testing

· Hypertension Clinic

· Travel immunisation

· Childhood immunisation

· Assisting GP with minor surgery

· Diabetes Clinic

· Contraception Counselling

· Health Screening and advice

 Care Navigator

Location: The Swan Practice, Buckingham and Steeple Claydon Delivering to the                               surrounding area within our practice boundary.

Contract type: Permanent

Hours per week: Full or Part Time

Pattern: To be confirmed

Payscale: Grade 2

The Swan Practice are looking to recruit a professional, friendly and motivated person who enjoys working with the public, to join our busy practice as a Care Navigator.

The successful applicant will have an interest in primary health care. They will have the ability to work flexibly as part of an integrated multi‐skilled team, as well as work independently. The applicant should be reliable, positive, and professional, with a non‐judgemental attitude and have good communication and interpersonal skills.

They will need to be able to offer flexibility to cover for colleagues’ planned absences and work overtime to meet essential work demands from time to time.

Working with a large team of Care Navigators across three surgeries you will be the first point of contact for our patients. You will be working alongside our clinical and administration staff ensuring that we offer an excellent level of patient service. You will be a good listener who is caring and empathetic, and have clear communication skills both face to face and using the telephone. You will be guided by the site Supervisors and Team Leaders and will work in accordance with the practice guidelines and protocols. The post-holder will be responsible for navigating our patients to the correct type of appointments and services the surgery has to offer.
