Doctor to text appointment time - November 27th 2019
Over the next few weeks, the practice is going to be testing a new system for patients who need to see a doctor or minor illness practitioner the same day. After having made the request to our care navigators the doctor will review the patients requirements and then, with the patient's consent, he/she may send a text message with an appointment time.
We presented this new arrangement to the Patients Participation Group at our last meeting and it was received with enthusiasm. If a patient does not wish to be texted, then they will be telephoned as usual.

Patient Information Used For Research and Planning
Following the recent national news stories we would like to reassure our patients that we take privacy very seriously and complies with the Data Protection laws and NHS standards in keeping information confidential and secure. The practice does not store data in the same way as the Trusts involved and if you have been treated at any of the Trusts listed in the recent new items then please contact them directly. You can be reassured that we do not share any identifiable information outside of direct patient care without your consent. If you want to opt out of your information being shared for purposes beyond your care and treatment you can register on the national data opt-out website https://www.nhs.uk/your-nhs-data-matters/ or call the NHS Digital contact centre: 0300 3035678 which will prevent your data being used by the NHS for research and planning.

Dr Amabelle White
It is with great excitement and sadness that the Partners wish to inform you that Dr Amabelle White left The Swan Practice on the 14th November.
Her and her husband, Ian, are expecting their first child in December and Amabelle will be relocating to the coast to be with Ian.
We wish her all the best for this next exciting chapter of her life.
She will be greatly missed.
Practice Closure - Practice Training Afternoon
The practice will be closed from 1pm, on the afternoon of Thursday 19 March 2020. If you require medical attention please call NHS111 by dialling 111. For all other enquiries, including prescription requests, please contact the practice when we open the following day.
Nurse Olivia is on-hand 24/7- Download the new app Ask NHS
The app is free to download on any mobile device and provides patients with a symptom checker, self-care advice and access to book the most appropriate GP appointment or direct you to NHS 111, if required.
Using the app can help you take more control of your own health. A virtual healthcare assistant, Olivia, will ask you a series of questions (approximately 15) to assess your symptoms in a clinically safe and NICE accredited way. Olivia will then recommend the most appropriate care for you which might be self-care advice via NHS Choices, appropriate GP appointment, GP call back or direction to urgent or emergency care.
For patients who need an appointment, the app can prioritise those patients into the most appropriate time slot based on their needs. You’ll be able to directly book an appointment without having to call the practice.
